Answer 8: What effect does high-dose levofloxacin have on duration of therapy?

A retrospective analysis evaluated whether high-dose levofloxacin shortened duration of therapy, using symptom resolution as the primary endpoint to indicate when to switch from intravenous to oral therapy. Results from 232 patients in the intent-to-treat population and 168 patients from the clinically evaluable population confirmed that the 750 mg levofloxacin regimen was associated with a faster resolution of fever compared with the 500mg dose. In the ITT population, patients receiving 750 mg levofloxacin switched from intravenous to oral medication at a median of 2.35 days, compared with 2.75 days for the 500 mg group (p = 0.098). These superior results for the 750 mg levofloxacin dose were confirmed in the clinically evaluable population. By Day 3, 68.1% of the 750 mg patients had been switched to oral therapy, compared with 61.0% for the 500 mg group. By Day 4, nearly 83% of the 750 mg patients had been switched compared with 76% in the 500mg group. In general, this more rapid intravenous-to-oral switch is associated with a reduction in length of stay of CAP patients (7).
